with the subject: Recovery_section=account_recovery

and the text:

#[your account password]#
#[the name of a character on the account you want]#
#[the same characters realm]#

it should look something like this:

If you send it, you will get an answer in about 24 hours (sometimes more and sometimes less) with the account name, and the account password.
